Understanding Gynecomastia
What Causes Male Breast Enlargement?
Gynecomastia โ enlarged male breast tissue, often described as 'man boobs' โ is a common and 100% treatable condition, most often caused by a hormonal imbalance between oestrogen and testosterone, though genetics, certain medications, and weight can also contribute.
It can affect one or both sides of the chest and is a common source of self-consciousness that limits clothing choices, swimming, exercise, and confidence more broadly. At Aakar Aesthetics, Nashik, Dr. Kiran Nerkar treats gynecomastia using liposuction, surgical excision, or a combination of both, depending on whether the enlargement is mainly fatty tissue, glandular tissue, or both.
Understanding Your Condition
We Specialize InGynecomastia Surgeries
โGynecomastia is a medical condition characterized by the enlargement of male breast tissue, often due to hormonal imbalances. Treatment typically involves surgical removal of excess glandular tissue and fat through excision and liposuction procedures.โ

Is It Really Gynecomastia?
Not all chest fullness in men is true gynecomastia โ some cases are simply fat accumulation (sometimes called pseudogynecomastia) that may respond to weight loss, while true gynecomastia involves glandular breast tissue that will not resolve without treatment. Before recommending surgery, Dr. Nerkar evaluates your chest tissue composition and, where relevant, may recommend hormonal/endocrine testing to rule out an underlying medical cause, particularly for one-sided (unilateral) enlargement.
Understanding Severity
Grades of Gynecomastia
The amount of glandular tissue, fat and excess skin influences which surgical technique may be most appropriate.
Grade 1
Small enlargement, no excess skin: usually treated with liposuction alone.
Grade 2
Moderate enlargement, no significant excess skin: liposuction, sometimes combined with limited excision of glandular tissue.
Grade 3
Moderate-to-severe enlargement with mild excess skin: combination of liposuction and excision.
Grade 4
Marked enlargement with significant excess skin, resembling a female breast shape: excision with skin removal, sometimes including nipple repositioning.
Your Treatment Journey
How Gynecomastia Surgery Works
The surgical approach depends on whether the chest enlargement is primarily fatty tissue, glandular tissue, excess skin or a combination.
Anaesthesia
IV sedation or general anaesthesia, based on extent of correction.
Liposuction technique
Your medical history, medications and relevant factors are reviewed. Hormonal or endocrine testing may be recommended where clinically appropriate, particularly for unilateral enlargement.
Excision technique
used where glandular tissue or excess skin needs to be removed, and if the nipple/areola needs repositioning or reduction; incisions are placed where the areola meets the chest skin to minimise visibility.
Combined approach
liposuction and excision are frequently used together for the most natural chest contour.
After Your Procedure
Gynecomastia Surgery Recovery
Recovery varies depending on the amount of tissue removed and whether liposuction, excision or both techniques are used.
Follow your surgeon's instructions for compression garments, wound care, medication, activity and return to exercise.
Day 1โ3
compression garment worn to support new chest contour and reduce swelling; drainage tube (if used) removed within days.
Day 3โ5
most patients return to light activity/work.
Week 3โ4
full recovery for most patients, with continued swelling resolution.
Following Months
โ Results are visible immediately, improving further as swelling resolves and incision lines fade.

Frequently Asked Questions
Gynecomastia Surgery FAQs
Answers to common questions about causes, treatment techniques, recovery and results.
Gynecomastia is most commonly associated with an imbalance between oestrogen and testosterone. Genetics, certain medications, hormonal conditions and changes in body weight can also contribute. Once glandular breast tissue has developed, it may not disappear with weight loss alone.
